The medical social worker will assess the psychosocial functioning, environmental and the support needs of patients and families. This may include connecting patients and families to necessary resources and supports in the community; supportive counseling, or helping to expand and strengthen the network of social supports. The medical social worker is part of the interdisciplinary team: nursing, physical therapy, occupational therapy and speech therapy.
